Can I use insurance to pay for nutrition counseling?
If you are purchasing a meal plan, it will not be covered by insurance.
For nutrition counseling, Bites With Blair, LLC is currently in-network with Blue Cross Blue Shield. Not all nutrition-related diagnoses are covered with each plan, and a physician referral is often required for coverage. If utilizing insurance, all insurance information must be provided at least 48 hours prior to the appointment for Bites With Blair, LLC to verify insurance and check your individual plan terms for coverage of nutrition services. Insurance is billed directly on your behalf by Bites With Blair, LLC if your coverage has been verified in advance of your visit. All applicable co-pays and/or deductibles will be due at the time of service.
In the event you have a deductible, we will not be able to initially bill your insurance company directly. Therefore, payment of $160 is due at the initial visit and $80 is due at each follow up visit. We will provide you will the appropriate documentation to submit to your insurance company to show receipt of the services. This will allow you to “pay down” your deductible. Once your deductible has been met and you have nutrition services on your policy, we can then directly bill your insurance company.
Note that not all insurance plans will cover nutrition services. Many insurance companies have additional requirements or stipulations that may affect your coverage. You are responsible for any amounts not covered or payable by your insurance. If your insurance denies any part of your claim, you agree to be responsible to pay the full balance and the credit card on file will be charged for services provided.
If insurance information is not provided at least 48 hours in advance, the cost of service must be paid in full by the client prior to the session. Bites With Blair, LLC will provide the client with a Superbill upon request, which the client can submit to their insurance company for possible reimbursement. (Instructions for doing so will be provided with the Superbill.)
In the event you carry insurance in which we are out of network, you will need to pay for your counseling session at the time of service. We are happy to provide you with a “Superbill” and/or an itemized invoice you can submit to your insurance company for possible reimbursement. Reimbursement is based on your policy’s standards. Please call your insurance company’s member services number to verify how reimbursement works under your current insurance policy.
What is your policy for missed appointments/cancellations/late arrivals?
In order to provide the most professional session for you, please arrive on time. If you arrive to your appointment late, the session will end at the scheduled time regardless of when it started and full payment is expected. In other words, your session will be shortened to accommodate others who have appointments following yours and you will be charged for the missed time.
There may be times you need to reschedule or cancel your appointment. If so, please notify me at least 24 hours in advance of your appointment – by phone or email – or you will be charged for that scheduled time (except in case of illness or emergency). If you do not attend your appointment or have not provided a 24-hour cancellation notice, you will be charged a fee of $50 to your credit card on file for initial appointments or a fee of $25 for follow-up appointments. Every client is required to provide a credit card to keep on file in the event of a missed appointment or late cancellation. Medical emergencies are eligible for exemption when evidence is provided.
How can I make an appointment?
You can make appointments through our online scheduling system, which can be found on the "Work With Me" tab. To ensure that we have the necessary paperwork and ability to verify insurance benefits, appointments must be scheduled at least 72 hours in advance.
Can I email you to ask nutrition questions?
Email may not be a confidential method of communication. Phone or face-to-face contact are the best ways to communicate personal information. Bites With Blair, LLC typically checks email daily but does not guarantee immediate response to email contact. Therefore, email should not be used for urgent matters. Please contact 911 or visit the nearest emergency room if you are feeling unsafe or experiencing a life-threatening emergency.
Bites With Blair, LLC does not offer nutrition counseling via email, but uses email communication mainly for scheduling purposes and invoices. Email is only to be used for concise communication of administrative issues such as rescheduling an appointment. Email will not be used to discuss the course of treatment and/or diet planning. If you choose to send or receive emails containing personal information, you take responsibility for the security of that information shared. Bites With Blair, LLC is not on-call 24 hours a day. However, you are free to call our office phone and leave a voice message at any time. Your call will be returned within 24 hours Monday-Friday. If a message is left on Friday, the call may not be returned until Monday.
Will my information remain confidential?
Yes, you will be provided with the HIPAA Notice of Privacy Practices form when you schedule an appointment and can request a copy of this form by emailing blairpersyn [at] gmail [dot] com.
